Mediese sertifikate

In terme van die wet op Basiese Diensvoorwaardes moet werknemers mediese sertifikate inhandig indien hulle vir meer as twee dae weens siekte of besering afwesig is. Siek notas kan ook versoek word indien werkers vir meer as twee geleenthede gedurende ʼn agt weke siklus nie by die werk kan wees nie.

Die mediese sertifikaat moet aandui dat die werker nie in staat is om sy dienste te verrig nie en moet onderteken wees deur ʼn persoon wat gekwalifiseer is om pasiënte te diagnoseer en te behandel en wat geregistreer is by ʼn beroepsraad wat by ʼn Wet van die Parlement ingestel is. Mediese sertifikate wat deur persone soos verpleegsters, tradisionele genesers en maatskaplike werkers uitgereik word is nie aanvaarbaar nie.  

Voorts bestaan daar soms verwarring oor presies watter inligting in ʼn mediese sertifikaat vervat moet word.

Die volgende uittreksel van die Etiese en Professionele Reëls van die Suid-Afrikaanse Geneeskundige en Tandheelkundige Raad verskaf riglyne oor presies watter inligting so ʼn sertifikaat moet bevat.

Reël 15.(1)   van die genoemde reëls bepaal dat ʼn Praktisyn slegs ʼn sertifikaat sal uitreik indien dit die volgende inligting bevat.
 
(a)   Die naam,  adres en kwalifikasie van die praktisyn;
(b)   Die naam van die pasiënt;
(c)   Die diensnommer , van die werknemer (indien van toepassing)
(d)   Die datum en tyd van die ondersoek,
(e)   of die sertifikaat uitgereik is as gevolg van persoonlike observasie deur die praktisyn gedurende die ondersoek, of as gevolg van inligting verskaf deur die pasiënt en wat aanvaarbaar is op mediese gronde;
(f)   ʼn beskrywing van die siekte, in leke taal met die toestemming van die pasiënt. Indien die pasiënt nie toestemming gee dat die siekte of mediese toestand geopenbaar word nie, sal die mediese praktisyn of tandarts bloot spesifiseer dat, in sy /haar opinie gebaseer op die ondersoek van die pasiënt dat die pasiënt ongeskik vir werk is;   
(g)   of die pasiënt heeltemal ongeskik vir werk is en of die pasiënt in staat is om minder  veeleisende of inspannende werk te kan doen;
 
(h)     die presiese periode waarvoor siekte verlof aanbeveel word;
 
(i)       die presiese datum waarop die mediese sertifikaat uitgereik is, en
 
(j)   ’n duidelike indikasie van die identiteit van die praktisyn wat die sertifikaat uitgereik het wat  persoonlik en oorspronklik deur hom/haar geteken is met sy/haar naam in blok letters langs die handtekening.
 
(2) Indien vooraf gedrukte skryfbehoeftes gebruik word, sal die praktisyn die irrelevante woorde wat nie van toepassing is nie skrap.
 
(3)   ʼn praktisyn sal ʼn kort feite verslag uitreik waar die pasiënt informasie versoek met betrekking tot hom/ haarself.
